To streamline your warranty exchange, please fill the form out below. It is a requirement to send the item back in to us for the warranty exchange. By filling out this form and providing a tracking number and photo of the broken item, we have the opportunity to work on your exchange request before we recieve the item.

We are hoping this will cut down on the time it takes for you to get the exchange. In some rare instances, shipments get misplaced and with this form completed, even if we have not recieved the package OR the event it gets lost in the mail, we can and will still ship your new item.
